Step 2: Complete and sign the form
Please review and electronically e-sign the form as soon as possible.
Step 3: Activation
Once your signed form is received and processed with the IRS, your transcript monitoring will begin

If you don't receive this email within 24 hours please contact our office directly. If you are unable to complete the form electronically, please contact our office and we will assist you with an alternative option. Monitoring does not begin until IRS authorization is completed.
